Offer valid from January 1 - June 30, 2026. To be eligible for offer, customer must purchase a new Welch Allyn Spot Vision Screener from School Health (*qualifying School Health part numbers: 1003001, 1006090, 1007189, 1035686, 1041132 and 1003012SP) or a Welch Allyn OAE (**qualifying part numbers #1007187, #1007188, #1007189) and provide proof of purchase and W-9 as indicated on the redemption form found at hillrom.com/schoolhealth2026-q1q2. All information must be submitted via the online redemption process by August 15, 2026. Valid for School Health customers in the U.S. only. Distributor stocking orders do not qualify. Please allow 12 weeks for receipt of rebate. Promotion cannot be combined with any other promotion, offer, coupon, enhanced discount or special pricing arrangement; some exclusions apply.
