Suvida Healthcare: A Model for Vision Preservation in Value-Based Care

Helping to Meet Quality Goals While Improving Access to Diabetic Retinal Exams with the Welch Allyn RetinaVue Care Delivery Model

When Suvida Healthcare opened its first clinic in January 2023, its mission was bold and clear: deliver culturally aligned, high-quality care to Hispanic seniors in Texas and Arizona. From the beginning, that meant making diabetic retinal exams a standard part of the care journey for every patient living with diabetes. “We knew diabetic retinopathy screening would be a key piece in our clinical model,” said Raymond Donato, MSN, APRN, AGPCNP-BC, Vice President, Clinical Operations. “But more importantly, we knew early detection could help preserve our patients’ vision. That principle has shaped our approach from the beginning.”

A Vision-First Approach to Value-Based Care

As a full-risk, value-based care provider, Suvida Healthcare is accountable for all aspects of patient care. That model calls for proactive, preventive care centered on the patient — anticipating needs before complications arise. The RetinaVue solution helps make that possible by integrating vision screening directly into everyday care. “We’re responsible for the total health of our patients, and that includes helping to prevent avoidable complications like vision loss in patients living with diabetes,” said Donato. “The RetinaVue care delivery model helps us do that efficiently, right at the point of care.”

Suvida Healthcare serves more than 11,000 patients across 13 locations. From day one, the RetinaVue solution was embedded into the practice — not as an add-on, but as a diagnostic tool part of its standard clinical workflows.

Workflow-Integrated, Data-Driven

Suvida Healthcare’s data-driven clinical model and infrastructure play a central role in helping to ensure no patient falls through the cracks. The team uses real-time data to track which patients living with diabetes are due — or overdue — for their annual diabetic retinal exam. If a care gap is identified, the exam is automatically built into the visit. “We assess patient needs for diabetic retinal exams during their appointments,” said Donato. “It’s just like checking vitals or doing an EKG — it’s part of the visit.”

Tracking Quality, Delivering Results

In 2024, Suvida Healthcare achieved Five-Star compliance for diabetic retinal exams. To date, Suvida Healthcare has already performed more than 2,500 exams in 2025 — completing exams on 86% of its target population. “We’re not just checking a box,” said Donato. “We’re using patient-level datainsights to drive action. Our clinical processes show us exactly who is missing an exam, and our teams follow up to help close those gaps.”

Scalable Solutions

Every new Suvida Healthcare clinic is equipped with a Welch Allyn RetinaVue 700 Imager from the very first day it opens. Making this technology available immediately reflects Suvida’s commitment to giving patients and care teams the resources they need to deliver high quality diabetes care.

As Suvida has grown across Texas and Arizona, it has also scaled strategically by expanding the number of cameras at existing locations as needed. Suvida closely tracks patient population at every center to ensure there is adequate coverage at all locations.

“We don’t want patients to leave without getting the care they need,” said Donato. “We’re constantly looking at our population health data and internal processes to make smart decisions about where and when to allocate additional resources.”

Why RetinaVue Works for Suvida Healthcare

For Suvida Healthcare’s patient population, social determinants of health come into play, which can be a major barrier to follow-up care. That’s why offering the diabetic retinal exam in-house is impactful.

“Our patients love that they can get the exam done right here, with the staff and providers they know and trust,” said Donato. “It’s fast, simple and fits into our clinical workflow without adding additional burden to our patients.” With RetinaVue, P.C. providing overread services, result reports are typically available the next business day, allowing providers to follow up promptly.

More Than a Clinic — A Community

Suvida Healthcare is more than a healthcare provider — it’s a community hub. In addition to medical care, patients come for weekly activities, social services and connection.

“We try to do as much as we can under one roof — physical therapy, dietary support, mental health services, clinical pharmacist support and social services,” said Donato. “Every service we can offer in-house is one less barrier for our patients.” As an organization, Suvida is constantly looking for ways to bring more care in-house, like diabetic retinal exams with the RetinaVue care delivery model.

“Our patients love that they can get everything done here,” said Donato. “Providing diabetic retinal exams supports the health of our patients by helping to preserve their vision, giving them one less thing to worry about.”
