Vision screeners eligible for trade in include: Welch Allyn SureSight Vision Screener, Welch Allyn Spot Vision Screener (2.0 model), PediaVision, GoCheck Kids, Plusoptix, MTI, Topcon, Retinomax, Titmus V Series, Stereo Optical Vision Screeners, Canon, Reichert, Marco and/or iScreen brands. Offer valid from January 1, 2023 - December 31, 2023. To be eligible for offer, customer must purchase a new Spot Vision Screener (model number VS100S-B or VS100SH-B) and provide proof of purchase, W-9, photo of trade-in device with serial number or UPC label visible as indicated on the redemption form found at hillrom.com/svsupgrade. All information must be submitted via the online redemption process by February 15, 2024. Offer cannot be combined with any other offer, discount or GPO contract pricing. Some exclusions may apply.
